I am running a small lottery syndicate at work. Does anyone know the wording of a short agreement?

When the lottery first started I am sure there was a simple statement that could be drawn up between collegues to detail the rules of the lottery syndicate, payments ect.I am running a small lottery syndicate at work. Does anyone know the wording of a short agreement?
make sure your agreement covers the following:



The date the agreement was drawn up;

The appointed manager's name;

The names of the members of the group;

The games and numbers to be played;

Which draws will be entered i.e. Wednesday, Saturday or both;

How much each member will pay per draw;

How any winnings will be split;

What happens if a member fails to pay their contribution at any time;

If the group has a big win, whether the members will take publicity or not.

The agreement must be signed and dated by each group member and should be witnessed by, we suggest, a solicitor, doctor or someone of similar standing. Copies should be given to each member and the original kept in a safe place; please don't send your agreement to us. If the group changes, the agreement should be amended to reflect this.
